On your FIRST VISIT to Dorr Chiropractic you will have some intake forms to fill out. This can be done in 1 of 2 different ways. Either is fine:

Click on the PDF link below. Download, print and fill the forms out at home. Then bring them in with you the first time you arrive
OR
​

Fill the forms out the first time you arrive at the office. The forms will be available and are hanging on the wall to your right immediately when you walk in. Simply, grab a clipboard with the forms, fill them out and give them to Mike when it's your time to be adjusted

-We frequently run specials where you can pre-pay for 4, 6, 8 or even 12 visits and save money on your adjustments
